Courtney joined 色色视频 in September 2025 as an account executive, helping grow behavioral health home care services throughout the Chicago market. For Courtney, this work is deeply personal.

Before joining 色色视频, Courtney spent nearly 15 years working in the cancer community. Throughout her career, one thing has remained constant: helping people find peace of mind during difficult times. When the opportunity to join 色色视频 came along, she knew it aligned with both her professional experience and her passion for helping others.

“As a cycle-breaker in my own family, this work resonates on a profound level,” Courtney said. “I couldn’t help but see a future that allowed me to bring my authenticity, my unique set of skills and my personality into the possibility of helping others find a little light during some very dark times.”

She said what she appreciates most about working at 色色视频 is the culture of collaboration and support. She values working alongside teammates and leaders who encourage innovation, celebrate successes and genuinely care for one another both professionally and personally.

“There’s a shared commitment to excellence without sacrificing work-life balance,” she said.

Some of the moments that mean the most to Courtney are helping patients overcome barriers to care, particularly Spanish-speaking individuals who may have difficulty accessing mental health services. Helping patients is what continues to motivate her.

When Jessica began her nursing career nearly two decades ago, she worked in a small community hospital where patient care looked a little different.

“It was old school,” she said. “It was slower paced, but still very respected by the community. It still had more of the one-on-one care and wasn’t so rushed.”

When the hospital closed its doors, Jessica found herself searching for a new path. She discovered skilled home health, where she could continue providing the personal, relationship-focused care that first inspired her to become a nurse.

Now, after 18 years in home care, Jessica still believes there’s no better place to care for patients.

“I have always been a field nurse,” she said. “Every time I think about trying something new, I realize how much I would miss being in the field. I would miss the interactions that I have with patients. Just by having these interactions, I continue to learn something new every day.”

As a licensed practical nurse (LPN) with 色色视频, Jessica visits patients in their homes, helping them recover from illness or surgery, manage chronic conditions and stay safely independent where they are most comfortable. She provides skilled nursing care while also educating patients and families so they feel confident managing their health between visits.

For Jessica, though, nursing has always been about much more than clinical care.

“What I do in home health is not all about medical,” she said. “It is also about family. When a patient shares something about their life, whether it is good or bad, sometimes they need a sounding board, and that’s what I can be for them, too.”

Over the years, those conversations and connections have become one of the most rewarding parts of her career. Patients often remember the compassion she brings into their homes just as much as the care she provides.

“I do love it when my patients come back to 色色视频 and ask for me specifically,” she said. “That makes me feel like I have done my job right.”

Jessica approaches every visit with the same philosophy that first drew her to nursing: treat people with compassion.

“Home health is about education, but it is also about kindness, caring and respect,” she said. “These things are what people are looking for.”

When Clara joined 色色视频 as a home health aide in 2013, she knew she wanted a career where she could help others. More than a decade later, that same passion helped her build a rewarding career supporting patients and families in a new way.

After spending years providing hands-on care in patients’ homes, Clara was promoted to care team supervisor in 2023. Today, she serves as an NHTD and TBI waiver program specialist, helping individuals access home and community-based services so they can continue living safely and independently instead of in a nursing facility.

The NHTD (Nursing Home Transition and Diversion) and TBI (Traumatic Brain Injury) waiver programs help eligible New Yorkers receive personalized care and support in their own homes and communities. Clara works closely with patients and families to guide them through the process, answer questions and make sure they receive the services they need.

In one instance, Clara worked closely with a family throughout the NHTD waiver application process, helping them understand each step and providing guidance from start to finish. When everything was complete, the family shared just how much her support meant to them.

“They complimented me for my assistance and said they couldn’t express their gratitude in words,” Clara recalled. “That moment allowed me to see the impact of my work and the value in it.”

Clara said the people she works alongside have also played a major role in her career satisfaction. She appreciates the teamwork, trust and encouragement she has found throughout her time at 色色视频.

“I love working at Elara because of the highly collaborative work environment and the culture of never leaving others behind,” she said. “The trust of my manager and the flexibility allowed in my schedule are what stand out to me.”

As Clara’s career has grown, so has her ability to help others. What began as providing one-on-one care has evolved into helping families navigate life-changing programs with confidence and compassion.

When Hemang joined 色色视频 as a PRN physical therapist in January 2023, he was looking for an opportunity to help patients where they feel most comfortable: their own homes. Just six months later, he transitioned into a full-time role, and he says he’s continued to grow every step of the way.

“I have never looked back and have enjoyed this great team and organization,” Hemang said.

As his career has grown, so has his confidence. He credits his teammates with helping him strengthen both his clinical skills and his ability to care for patients facing complex health challenges.

“This organization helped me build a strong foundation in my professional skills, where I learned the importance of teamwork, problem-solving and consistency,” he said. “This team helped me become more confident and handle complex situations.”

That support has encouraged Hemang to keep learning, take on new responsibilities and look for opportunities to mentor others in the future.

His passion for physical therapy comes from seeing the difference it makes in people’s everyday lives. While helping patients recover physically is important, what motivates him most is helping them regain their independence and confidence.

“I love being a physical therapist because it allows me to make a direct impact on patients in their everyday lives,” he said.

Providing skilled home health physical therapy gives Hemang a perspective that’s difficult to find anywhere else. By seeing patients in their own homes, he can better understand the obstacles they face and create treatment plans that fit their daily routines and personal goals.

“Treating patients in their home environment gives me a better understanding of their real challenges and allows me to create more personalized, functional treatment plans that truly improve their independence and quality of life,” he said.

Every home is different, and every patient has a unique story. That’s one of the things Hemang enjoys most about home health.

“Every home and patient situation is different, so it keeps the work engaging and rewarding,” he said. “It allows me to combine clinical skills, patient education and compassion to help people stay safe and comfortable in their own homes.鈥

One patient in particular continues to remind Hemang why he chose this career. He cared for a veteran with bilateral amputations who had lost confidence in his ability to move safely after surgery. At the start of care, everyday activities like rolling in bed, getting dressed and safely transferring to the commode felt overwhelming for both the patient and his family.

Over the following weeks, Hemang focused on improving strength, balance and mobility while helping the patient believe in himself again. By the end of care, the patient was walking safely, participating in more daily activities and looking toward the future with renewed confidence.

“What made me proud wasn’t just the physical progress,” Hemang said. “It was seeing how improved mobility positively affected his overall quality of life and reduced stress for the family as well.”

Moments like that continue to inspire him every day because they show that physical therapy is about more than movement. It’s about helping people reclaim their independence and giving families peace of mind.

Looking ahead, Hemang hopes to continue building on what he’s learned while helping others do the same.

For nearly 15 years, Jeniffer has helped people receive the support they need to remain safe, comfortable and independent at home. Today, as a Personal Care 色色视频 Regional Branch Director, she oversees multiple locations and teams, but her passion for home care has remained the same since the beginning.

Jeniffer joined the organization in 2011 as a Coordinating Manager, supporting Medicaid personal care services. Over the years, she took on new responsibilities, including managing private pay services, helping grow community partnerships and leading multiple branch locations. In 2018, Jeniffer stepped into her current role as Regional Branch Director.

“This August, I will proudly celebrate 15 years with the organization,” Jeniffer said. “Throughout my journey, I have been fortunate to grow professionally while helping others along the way.”

Her career has been built on a simple belief: quality care starts with people.

From patients and families to caregivers and office staff, Jeniffer has spent her career creating connections that help others succeed. While many people think of home care as the support patients receive, Jeniffer has seen firsthand how important it is to support caregivers as well.

“One of the most rewarding parts of my career has been building strong and meaningful relationships with caregivers and patients,” she said.

As a bilingual leader, Jeniffer has been able to connect with Spanish-speaking caregivers and help create opportunities for professional growth. She has worked to recognize talent, encourage development and help individuals build careers in healthcare.

“Being bilingual in Spanish allowed me to connect with caregivers and help create opportunities for them to grow within healthcare,” Jeniffer explained. “Many caregivers were able to transition into different service lines based on their abilities and strengths.”

That commitment to people has helped strengthen the care experience for patients and families while building high-performing teams that deliver exceptional care at home.

For Jeniffer, one of the most rewarding parts of home care is seeing the lasting relationships that develop over time. Unlike many healthcare settings, home care allows caregivers and care teams to become part of a patient’s journey, often creating bonds that continue long after services end.

“What I enjoy most about working here is that so many people share the same passion, dedication, energy and commitment to providing exceptional care at home,” she said.

After years of serving patients across her community, Jeniffer has accumulated countless memories. Some come in the form of holiday cards from former patients and families. Others come from moments that remind her just how much human connection matters.

One memory stands out in particular.

“A longtime paraplegic patient who rarely left his home except for medical appointments arranged transportation to visit our branch office personally just to meet me,” Jeniffer recalled.

It was a simple gesture, but one that left a lasting impression.

“Being part of something that can positively change someone’s life is truly an incredible feeling,” she said.

As she approaches 15 years with 色色视频, Jeniffer remains focused on supporting patients, families and caregivers while helping ensure more people can receive high-quality care where they are most comfortable: at home.

At 101 years old, Tolley has lived through some of history’s most significant moments. A World War II veteran, D-Day survivor and Senior Olympics champion, he has spent more than a century proving that determination and hard work can carry a person a long way.

His story is one of service, resilience and an unwavering commitment to staying active and independent.

Recently, 色色视频’s Hammond, Louisiana team had the privilege of providing skilled home health services to Tolley. Physical therapist Casey worked with him through the Better Balance program to help maintain his balance, mobility and independence at home. Supporting veterans like Tolley is part of 色色视频’s Front & Center initiative, which recognizes the unique needs of those who have served.

And if you ask Tolley, staying active is simply part of who he is.

Every day, he completes his home exercise program twice and walks 15 laps through his home. When his home health services conclude, he is already looking forward to returning to his weekly exercise class.

That commitment to staying active has been a constant throughout his life.

At 17, Tolley joined the Navy. Two years later, he was serving as a gunner’s mate during the D-Day invasion on June 6, 1944. Today, he is the only living Louisianian known to have participated in the invasion. Visitors can even hear his story through the Voices from the Front exhibit at the National WWII Museum.

After returning home from military service, Tolley continued to embrace an active lifestyle. He built his own home and barn in his 50s with the help of his wife and son. In his 70s, he discovered the Senior Olympics after seeing a flyer at a Tai Chi class. What started as a curiosity quickly became a passion. Over the next 15 years, he competed in events and earned nearly 100 medals.

One accomplishment still stands out to him today: running the 100-meter dash in 18.3 seconds at age 79.

When asked about his age, Tolley said he doesn’t spend much time thinking about it. In fact, he points out that he isn’t even the oldest member of his family 鈥 his sister is 103.

For Casey and the Hammond team, working with Tolley has been an honor.

His commitment to maintaining his independence is evident in everything he does, from completing his exercises each day to staying active and engaged in the things he enjoys. Through skilled home health services and the Better Balance program, the team was able to support those goals while helping him continue living safely and confidently at home.

Tolley’s story is a reminder that age does not define what is possible. His resilience, work ethic and positive outlook continue to inspire everyone who meets him.

Vincenza’s home care career began in 2007 as a field nurse, a role that provided the foundation for the clinical leadership position she holds today. Along the way, she has built a career focused on patient advocacy, clinical excellence and helping people remain safe and independent at home.

During her time in the field, Vincenza developed strong clinical skills while building relationships with patients and families. One of the most rewarding parts of the role was seeing how consistent care and support could help patients improve over time.

“Seeing a patient smile, knowing that my care provides comfort and reassurance, is incredibly rewarding,” Vincenza said. “That continues to motivate me and gives me a strong sense of purpose.”

As her experience grew, so did her responsibilities. Vincenza became an in-service trainer, helping personal care aides strengthen their skills through both classroom education and hands-on support in patients’ homes. The role allowed her to share her knowledge while helping caregivers provide even better care.

In 2018, she transitioned into the role of clinical team manager. Today, she uses the experience she gained in the field to support patients, families and caregivers in a different way.

“My experience as a field nurse allows me to assess situations effectively, make sound clinical judgments and implement appropriate interventions,” she said. “I remain committed to ensuring that every patient receives the highest quality of care.”

For Vincenza, nursing has always been about helping people through health challenges and making a meaningful difference in their lives. Watching patients make progress is what continues to inspire her.

“I consistently reinforce that the patient is at the center of everything I do,” she said.

That commitment was especially evident when she worked with a patient who had been admitted to a nursing facility after experiencing repeated falls and hospitalizations. The patient wanted to return home but needed a higher level of support to do so safely.

Drawing on her knowledge of New York’s Nursing Home Transition and Diversion (NHTD) Waiver Program, Vincenza worked closely with the patient’s daughter and the care team to explore available options. Through those collaborative efforts, the patient was approved for additional services and able to return home with around-the-clock support.

Today, the patient is living safely in familiar surroundings with the care she needs.

“This experience was incredibly meaningful to me,” Vincenza said. “It reinforced my commitment to patient advocacy and care coordination, and I take pride in knowing that I helped connect the patient with the right level of care.”

Vincenza credits much of her own growth and success to the support she has received throughout her career.

“A significant part of my growth as a home care nurse is attributed to my manager,” she said. “Her guidance, patience and leadership have played a critical role in shaping the nurse I am today.”

She also values the teamwork that makes quality patient care possible.

“I am part of a dedicated and supportive team that includes both clinical and administrative staff,” she said. “Our teamwork and mutual support enable us to achieve positive patient outcomes.”

Today, Vincenza remains focused on advocating for patients, supporting her team and helping people receive the care they need to live safely and independently at home.

Helping patients regain strength and confidence at home is what drives Brian each day as a physical therapist with 色色视频.

Brian joined 色色视频 in April 2023 and quickly found purpose in the relationships he builds with patients and families during some of their most challenging moments. As a home health physical therapist, he works closely with patients recovering from illness, injury or changes in mobility, helping them safely move forward in the comfort of home.

鈥淢y 鈥榳hy鈥 comes from a passion for helping patients regain independence and improve their quality of life in the comfort of their own homes,鈥 Brian shared. 鈥淚 chose this career because I value building meaningful connections while making a tangible impact on patients鈥 lives.鈥

Since joining 色色视频, Brian has continued growing both personally and professionally. Managing a wide range of patient needs while collaborating with interdisciplinary care teams has strengthened his confidence and expanded his clinical skills.

Amanda has spent three decades caring for others, and she still finds joy in helping people through life鈥檚 most important moments.

Since joining 色色视频 in January 2025 as an LPN Clinical Coordinator, she has become part of the team working behind the scenes to help patients receive compassionate care in the comfort of home.

鈥淚 have been an LPN for 30 years, and I truly enjoy helping and caring for others in the good times and the bad times,鈥 Amanda shared.

One of the things she values most about home health is the opportunity to help patients remain in familiar surroundings while receiving the support they need to stay safe and independent.

鈥淚 love working for 色色视频 because we get to keep our patients safe in their homes where they can thrive at their best,鈥 she said.

In her role as a Clinical Coordinator, Amanda helps keep communication flowing between care teams, office staff, patients and families. She says the strong sense of teamwork across the branches plays a major role in creating a positive experience for everyone involved.

鈥淥ur team works together in all aspects 鈥 whether it be for other team members or for our patients 鈥 and we support each other daily,鈥 she explained.

That collaboration extends far beyond one department. Amanda is proud of the way every team member contributes to making patients feel valued, respected and cared for like family.

鈥淚鈥檓 proud that we as a team provide our patients with the best care,鈥 she said. 鈥淲hether it be the field staff or the office staff, we strive to make our patients feel as important as they are and treat them like family.鈥

While she spends each day supporting others, Amanda says she has also felt encouraged and uplifted by the people around her since joining 色色视频.

鈥淭he support and guidance I have received while working at 色色视频 has been amazing,鈥 she said. 鈥淚t gives me great joy knowing that I work for such an amazing company.鈥

After 30 years in nursing, Amanda still approaches her work with the same compassion that first led her into health care: helping patients feel supported, cared for and never alone in their journey.

Kyle鈥檚 passion for helping people shines through every part of his work as a speech-language pathologist. From helping patients regain communication skills to supporting safer swallowing at home, he has spent the last decade building meaningful relationships and guiding patients through their health journey.

鈥淚 chose this career because, at my core, I love helping people,鈥 he explained. 鈥淚 come from a competitive sports background, and that mindset has never left me. I have always viewed myself less as a clinician and more as a coach.鈥

That coaching mindset helps Kyle build strong relationships with patients and families while encouraging them through recovery and rehabilitation.

鈥淲hat brings me the greatest joy as a speech-language pathologist is getting to truly know my patients,鈥 he said. 鈥淏uilding genuine rapport, making real connections and understanding what drives them. There is nothing more rewarding than helping someone reach a goal they once thought impossible.鈥

For Kyle, some of the most meaningful moments are often the simplest ones.

鈥淭he expression on a client鈥檚 face when they pass a swallow study and are clear to eat by mouth again, or the moment a patient who has struggled to communicate is able to pick up the phone and have a real conversation with a friend or family member,鈥 he shared. 鈥淏eing the person who helps give that back to someone is something that never gets old.鈥

Kyle joined 色色视频 10 years ago. Since then, his role has grown beyond home health patient visits. He has contributed to several company-wide initiatives focused on improving care for patients receiving speech therapy at home. He said some of his proudest accomplishments have come from helping create tools and programs that improve the experience for both clinicians and patients.

鈥淢y role has expanded, becoming a team member for various company-wide projects,鈥 Kyle shared. 鈥淢ost notably 鈥楶roject Lightning,鈥 which has vastly improved our documentation for our speech pathologists throughout Elara, and the implementation of Constant Therapy, which has been a game changer for our patients with cognitive and language deficits.鈥

Kyle credits the strong culture and teamwork at 色色视频 as a major reason he has stayed with the organization for so many years.

鈥淲hat I love most about working at Elara is the people,鈥 he said. 鈥淢any of our therapists have worked together for 5, 10, even 15 years, which is truly rare in healthcare. We feel supported, we feel heard and there is a genuine commitment to positive change and innovative thinking.鈥
