Can You Change Hospice Providers? What Families Should Know

Choosing hospice care is one of the most important decisions a family can make. You trust a hospice team to provide comfort, support and guidance during a difficult time.

But what happens if the care you’re receiving isn’t meeting your expectations?

Many families are surprised to learn that hospice is not a one-size-fits-all service. If you feel your loved one’s needs aren’t being met, you may have the option to transfer to a different hospice provider.

Understanding your rights and knowing what to expect can help you make the best decision for your loved one and your family.

Can You Switch Hospice Providers?

Yes. Patients receiving hospice care through Medicare can transfer to a different Medicare-certified hospice provider.

Changing hospice agencies does not mean your loved one has to stop hospice care. Instead, it allows you to continue receiving hospice services through a different provider.

The goal is simple: ensuring patients receive the care, support and experience that feels right for them.

Why Families Consider Changing Hospice Providers

Typically, the patient or their representative chooses a new hospice provider and works with both agencies to coordinate the transition.

A transfer generally includes:

  • Selecting a new hospice provider
  • Completing the required transfer paperwork
  • Choosing an effective transfer date
  • Sharing medical records and care information
  • Coordinating medications, equipment and services to avoid interruptions in care

The new hospice provider will help guide families through the process and explain any required paperwork.

Will Hospice Care Be Interrupted?

One of the biggest concerns families have is whether there will be a gap in care. In most cases, hospice providers work together to coordinate a smooth transition.

If you are considering a transfer, ask the new provider how they will coordinate medications, equipment delivery and visit schedules during the transition.

  • How do you communicate with family members?
  • What support is available for caregivers?
  • What bereavement services do you offer?
  • How do you help manage symptoms and changes in condition?
  • How do you coordinate care with physicians and other healthcare providers?

The answers can help you determine whether a provider is the right fit for your family’s needs and preferences.
