Hospice Care at Home

Comfort, Support and Dignity When It Matters Most

Hospice Care

When facing a serious illness, the focus often shifts from curing the disease to making each day as comfortable and meaningful as possible. Hospice care at home provides compassionate end-of-life support in the place most people feel safest: home.

Our in-home hospice care is centered on comfort, symptom relief and caring for the whole family. We meet you where you are, physically and emotionally, and walk alongside you every step of the way.

What Is Hospice Care?

Hospice care is specialized end-of-life care for people living with a life-limiting illness who have chosen to focus on comfort rather than curative treatment.

Instead of more hospital visits and aggressive procedures, hospice brings expert medical care, pain management and emotional support directly to your home.

Hospice may be right if you or your loved one:

  • Have been told the illness may be life-limiting, often with a prognosis of six months or less
  • Are experiencing more frequent hospital or ER visits
  • Have increasing pain, shortness of breath, weakness or weight loss
  • Feel ready to focus on quality of life instead of ongoing treatments
  • Need more support than family alone can provide

Hospice care can be provided in a private home, assisted living community or nursing facility — wherever you call home.

How Hospice Care Helps Families

Hospice is not just for the patient; it is family-centered care.

We help caregivers feel informed and supported by:

  • Explaining what to expect as the illness progresses
  • Teaching how to provide safe, comfortable care
  • Being available 24 hours a day for questions or concerns
  • Offering emotional support during difficult moments
  • Providing grief support for up to 13 months after a loved one’s passing

Our goal is to reduce fear, ease stress and help families focus on time together.

For many families, cost is a concern. The good news is that hospice care is covered under the Medicare Hospice Benefit. Medicaid and most private insurance plans also provide hospice coverage.

Our team will explain your hospice benefits clearly and handle the paperwork for you.

Starting hospice care at home is easier than many people think.

  1. Talk with your doctor about whether hospice is appropriate or call us directly.
  2. We review eligibility and coordinate with your physician.
  3. Your hospice team begins visits and provides ongoing support.

We take care of the details so you can focus on comfort and meaningful moments.

Frequently Asked Questions

Everyone has a right to choose their healthcare options. If a patient wants to stop the hospice benefit for any reason, they can. Also with the increased care of a hospice team of providers, sometimes patients improve. If a physician believes the patient will live beyond six months, hospice can be discontinued. If or when the patient’s condition begins to deteriorate, the doctor can reassess, and the patient may become eligible once again.

The simple answer is as often as needed. The frequency of visits is based on the patient’s individual care plan and will increase or decrease depending upon the patient’s needs.

ɫɫÊÓƵ is a Medicare-certified hospice provider and is fully staffed 24 hours a day, 365 days of the year—including weekends and holidays. Patients and caregivers are encouraged to call any time and will be connected with the appropriate staff person based on the need. A nurse will visit to address a patient’s changing needs even if the visit is outside of the scheduled care plan.

Hospice is a program of care and support for individuals when a cure is no longer likely or the patient has chosen not to pursue aggressive curative treatment. The focus is on the patient’s quality of life and comfort. Hospice care is for the patient as well as the family and caregivers. Palliative care is a medical specialty focused on the needs of people who are living with serious, often life-threatening illness. The focus is on treating uncontrollable symptoms.

Following are some of the signs that it may be time for the added benefits of hospice:

  • Hospitalizations over the last year
  • Recent weight loss over the last six months
  • Difficulty swallowing
  • Needing more assistance with activities of daily living
  • Increased or new onset of incontinence
  • Decreased stamina
  • Increased edema
  • Increased shortness of breath
  • Decreased ability to communicate
  • Increased disorientation or confusion
  • Withdrawal from family and friends
  • Decreased appetite
  • Infections, such as UTI or respiratory
  • New wounds or wounds that do not heal
