Eligibility Criteria for NHTD
To qualify for the NHTD Waiver Program, individuals must meet the following criteria:
- Be eligible for Medicaid in New York State.
- Must have a Patient Review Instrument or PRI completed to determine level of care. This is essentially a 鈥減icture鈥� of the patient and determines the patient鈥檚 long-term needs.
- Patient must be Custodial Level of Care or Nursing Home Level of Care.
- Be able to live safely in a community setting with the support of waiver services.
