Interactive Patient Guide
An interactive kidney disease workbook designed to promote education, conversation and engagement between patients, caregivers and clinicians. Content focuses on symptom management, disease education, wellness and knowing when to take action.
Components of the Program
Specific zones are customized for a聽kidney disease diagnosis so patients and caregivers know when and how to take action.
Our centralized clinically led Engage Team provides personalized telephone touchpoints between visits to help answer questions, provide encouragement and offer additional support.
We are just a phone call away 24 hours per day, 7 days per week. If you have a question or concern, we want you to call us for help. We are ready to listen, answer questions, provide direction and take action whenever you need us.
Meet Paul
Paul is 72 years old and has been living with chronic kidney disease for several years. With help from his wife, he has been managing dialysis appointments, medications and the many daily routines that come with kidney disease. Over time, keeping up with everything started to feel more difficult.
After a recent fall, Paul was hospitalized with an infection and electrolyte imbalance. Following his hospital stay, his physician recommended 色色视频鈥檚 Kidney FOCUS skilled home health program to provide additional support at home.
A skilled nurse began visiting Paul regularly to monitor his health, check for signs of fluid buildup and help care for his dialysis access site. She also helped Paul and his wife better understand his medications, recognize warning signs and know when to call for help if something changed.
Physical therapy focused on helping Paul rebuild strength and endurance after his hospitalization. Through exercises tailored to his needs, he worked on improving balance, reducing fatigue and building stamina so he could more comfortably attend dialysis and medical appointments.
Occupational therapy helped Paul manage everyday tasks that had become more challenging, like getting dressed and moving around safely at home. His therapist introduced simple techniques and helpful adjustments to make daily activities easier while helping him conserve energy and maintain independence.
With added support, education and personalized care at home, Paul felt more confident managing his condition and staying safely at home.
