Meet Dan

Dan is 66 years old and has been living with Type 2 Diabetes for several years. He enjoys doing crossword puzzles and spending time with his grandkids.

Lately, his health has been getting worse. His vision has become more blurry, making it hard to see his puzzles. He feels weak and has numbness and tingling in his legs. He also has a sore on the bottom of his foot that isn鈥檛 healing, which makes walking painful and difficult. Because of this, he鈥檚 been less active and hasn鈥檛 been able to enjoy time with his grandchildren like he used to.

Dan hasn鈥檛 always followed a diabetes-friendly diet, and he isn鈥檛 sure how to properly care for his feet or track his blood sugar. His daughter noticed these changes and scheduled a doctor鈥檚 appointment. After the visit, his doctor recommended in-home care to help him better manage his diabetes and prevent further problems.

A nurse came to Dan鈥檚 home to check on him and teach him about diabetes and how to take better care of himself. Through the BLOOM program, he is learning how to manage his insulin, eat healthier, care for his feet and treat his wound so it can heal properly.

A physical therapist is helping Dan improve his strength and walking so he can move around more safely and reduce his risk of falling. An occupational therapist is also helping him adjust to vision changes, making daily activities like reading and puzzles easier and safer.

With this support at home, Dan feels more hopeful and in control of his health. He is focused on healing his foot, gaining strength and getting back to the activities he loves. His daughter also feels relieved knowing he has the care and guidance he needs to stay safe at home.
