PCS Engage is a program that allows caregivers to proactively alert the team when they notice changes in a personal care client鈥檚 health, helping ensure concerns are addressed quickly and care stays coordinated.
We stay alert to changes so you don鈥檛 have to worry alone. If you go to the hospital or something doesn鈥檛 feel right, we鈥檙e set up to hear about it quickly and step in when needed.

At the beginning and end of every visit, your caregiver answers a few simple questions about how you鈥檙e doing. This helps us automatically keep track of any changes in your health and quickly notify the care team if extra support may be needed.
Caregiver supervisors receive the alerts from your caregivers.
Our actions will depend on how serious the type of health change.
We might send a team supervisor or nurse out to check on you in person, or advise you visit your doctor.
Our goal is to help you address small health changes before a major decline occurs.
If a serious health concern is discovered, more drastic action may be advised.
