What is Assist?

Assist helps identify skilled home health patients who may benefit from additional support through personal care services.

You don鈥檛 have to manage everything on your own. Personal care services can provide extra help with everyday tasks like bathing, meal preparation, light housekeeping and other daily activities so you can stay safe and comfortable at home.

In your home environment, your Skilled Home Health nurse gets to know you more personally – understanding how you’re doing each day and helping make sure the right support is in place. They often can see when you’re struggling with getting dressed, keeping up with housekeeping and just getting exhausted from daily tasks.

Personal Care 色色视频 (PCS) bring more comfort, more ease, and more personalized support into the place you call home. With a focus on your daily needs and routines, PCS helps create a safe, nurturing environment where you can feel confident, cared for, and truly supported every day.

Meet Albert

Albert has been receiving skilled home health to help manage his COPD safely at home.

Rose, his 色色视频 nurse, has been helping with medications, symptom management and energy conservation.

Albert told Rose that he used to enjoy cooking, but now even standing at the stove leaves him short of breath. Household chores are becoming more overwhelming, and he has been skipping showers since he sometimes gets dizzy and is worried about falling.

Rose identified a need for additional support with daily living activities and initiated Assist, a pathway designed to connect patients to personal care services to enhance safety and independence.

A Care Coordinator reached out to Albert to listen to his specific concerns, educate on the opportunity for added care and connect him to more care to meet his changing needs. He learned they could help with meal preparation, light housekeeping, shower assistance, morning dressing and even companionship.

Albert didn鈥檛 want to admit he needed help at first but decided he did like the idea of 鈥渢rying it out part-time.” He didn鈥檛 want someone 鈥渢aking over his life鈥 but also realized that maybe he didn鈥檛 have to do everything alone. He decided that getting a little extra help now might mean staying independent longer.

 

 

How it's going.

Albert decided to give personal care services a try and now says he doesn鈥檛 know how he managed without them. He was even able to get care a few days a week by a fellow veteran through the Operation Veteran Care program.

Albert has continued to improve and is packing for his upcoming Army reunion out of state. He was discharged last week from home health, but his personal care services caregivers were able to stay in place. He likes that he can now spend his energy on things that matter to him and feels more safe when he showers, just knowing he鈥檚 not alone.
